The physical examination time

Physical examination arrangements

(1) Physical examination hospitals

The physical examination hospitals are designated by the county (city, district) admissions committee to provide Class A or above hospitals or corresponding hospitals. The main examining hospital must make corresponding, standardized and accurate conclusions on the candidates' physical examination in accordance with the requirements of relevant documents, and be responsible for its authenticity. Physical examination conclusions made for candidates by non-designated medical institutions are invalid.

Medical staff participating in the physical examination for admissions must have a strong sense of responsibility, a decent style, and familiarity with the business. They must be trained to carry out their work. Untrained personnel are not allowed to undertake this physical examination task. The chief examining physician should be a doctor with a title of deputy chief physician or above. Doctors who have children or immediate family members taking the college entrance examination that year should be avoided.

(2) Physical examination items

Physical examination items shall be carried out in accordance with the "Guiding Opinions". (3) Physical examination conclusions

Each county (city, district) education examination admissions agency should develop methods for informing candidates of their physical examination conclusions. Before the examination, candidates are reminded to pay attention to the conclusions or opinions of the medical examiner on their various physical examinations during the physical examination; if the medical examiner really needs to correct a certain physical examination result due to an error in filling in the form, the doctor needs to modify it in his own handwriting and sign for confirmation. Correction fluid is not allowed to be used for correction; after all items have been checked and the conclusions are clear, the staff should check whether all items in the physical examination form have corresponding conclusions. After confirming that there are no missed items or missed items, they should supervise the candidates to read and inform them of their physical examination results and Sign and confirm on the physical examination form. If the candidate raises objections to a certain physical examination item, he or she can apply for a reexamination within the specified time to the education examination enrollment agency in the county (city, district) where the registration is located. (4) Physical examination form management

1. The candidate number, candidate name, candidate photo, etc. on the physical examination form shall be printed uniformly by the admissions office of each county (city, district). Liver function test sheets must not be pasted on the back of the physical examination form and placed directly in the candidate's portfolio.

2. The hospital seal and doctor’s signature for physical examination must use blue ink pad.

3. The physical examination hospital (station) must establish an inspection and acceptance team.

The inspection and acceptance team shall conduct acceptance inspections of the candidates' physical examination forms one by one, focusing on checking whether there are incorrect items, missing items, unclear expressions, physical examination conclusions that are inconsistent with the actual results, missing or unclear seals, etc. filled in by the doctor.

4. The admissions department is responsible for pasting the barcode on the physical examination form. The barcode must be pasted correctly along the barcode area, with the name end facing the left side of the form and the candidate number end facing the right side of the form. When pasting, it must not exceed the barcode area and must not cover the black synchronization area.

5. Cadres who participate in physical examinations are required to be careful and have a strong sense of responsibility. Storing and transporting physical examination forms requires dedicated personnel and dedicated vehicles to transport them.

6. The scan of the physical examination form will be arranged after the college entrance examination. The specific arrangements will be notified separately.

(5) Relevant requirements for disabled candidates to apply for reasonable accommodation
