During the construction, it is found that there are cracks in the original xx computer room, which will cause great hidden dangers to the radiation protection. We actively cooperated with the hospital logistics support department to contact Jiangsu Haiming and Jiugang Design Institute, made a serious analysis and survey on the causes of cracks in the original computer room and the safety of the building, and put forward a feasible remedial plan for the radiation protection at the crack, supervising the reasonable, standardized and strict construction of the engineering construction unit. Now the civil engineering of the accelerator project has been basically completed and has the conditions for installing the machine.

2. Formulate and improve various rules and regulations of radiotherapy department, and formulate the management system, departmental responsibilities and post responsibilities, equipment operation procedures, safety operation system, equipment maintenance system, emergency plan for radiation accidents in Jiuquan Iron and Steel Hospital, and medical accident prevention system of radiotherapy department. The division of labor and the implementation of responsibilities for the three posts in radiotherapy department were clearly defined.

3. according to the relevant regulations of the provincial environmental protection bureau on the radiation diagnosis and treatment work in hospitals, the hospital must pass the "environmental impact assessment" of the provincial environmental protection bureau, and after confirming that the accelerator project is harmless to the surrounding environment, it can carry out the project construction and apply for the "radiation safety license" to legally carry out the radiation work. After receiving the task, Radiotherapy Department actively contacted the relevant departments of Provincial Environmental Protection Bureau, reviewed the drawings of accelerator civil project in our hospital, provided various materials, assisted the relevant departments of Provincial Environmental Protection Bureau to complete the Environmental Impact Assessment of Accelerator in xx Hospital, and passed the expert review meeting on the environmental impact of accelerator project in Jiugang Hospital organized by the Evaluation Center of Provincial Environmental Protection Bureau, which laid the necessary legal foundation for the legal and standardized radiotherapy work in our hospital. 4. According to the relevant regulations of the provincial CDC on the radiation diagnosis and treatment work in hospitals, the hospital can only carry out the radiation treatment project through the "radiation protection evaluation" of the provincial CDC and the "radiation protection pre-evaluation" organized by the accelerator project after it is confirmed that it is safe for the staff and the surrounding public. After the project is completed, it must pass the "radiation protection control effect evaluation" before it can legally carry out the radiation treatment work. The Radiotherapy Department actively contacted the relevant departments of the Provincial Center for Disease Control and Prevention, and seriously welcomed the "radiation protection pre-evaluation" organized by the relevant departments of the Provincial Center for Disease Control and Prevention. In a short time, more than 2 kinds of materials such as various rules and regulations required by the "evaluation" were formulated and completed. second, subjectively, the members of different departments have different levels, the good and the bad are intermingled, and they are fragmented, which will inevitably affect their work. First of all, the team consciousness is not strong, and the atmosphere of helping and learning from each other needs to be improved. When breast cancer or body patients with multiple target areas are treated, the order of target areas on the treatment list is inconsistent with the order of target areas on the control computer, and the setting is based on the conditions of the treatment list, while the plan is executed in the order on the computer;

4. The patient's name is similar or the same, and the plan opened by the technician does not match the patient who entered the computer room.

the way to effectively avoid similar situations is to strictly implement the check system. First of all, before the treatment of each patient, it is necessary to check whether the patient's fixing device is consistent with the patient's information (the information on the patient's fixing device includes the patient's name, bed number, treatment method, pillow number, etc.). Secondly, we must carefully check whether the patients entering the computer room are consistent with the plan transferred out. The technician in charge of setting must let the patient who enters the computer room name, and hand over the patient's treatment list to the technician in charge of calling out the plan for review, strictly implement the system of one person and one list, and require that the treatment information on the treatment list must be carefully checked before treatment, and it can be released only after it is confirmed, and double-signed for confirmation. Don't take another patient's treatment list to sign in advance before the current patient's treatment is completed.

3. Understand the physical condition of the patient being treated. If the patient's physical condition is obviously unwell, such as bleeding, vomiting, dizziness, etc., immediately tell the patient to temporarily stop the treatment. If the patient insists on the treatment, please closely observe the patient's reaction. Once it is found inappropriate, immediately stop the treatment and enter the computer room to release people.
