Recently, some residents in our district have experienced changes in the Luoyang Electronic Health Code. To do a good job in people's health management services in the district, in accordance with the "Notice of the Luoyang City New Crown Pneumonia Epidemic Prevention and Control Headquarters Office on the Health Code Change Rules" and other requirements, the residents in our district are now notified of the changes in the Luoyang electronic health code as follows: :

The Sheluo Health Code is a yellow code. Those who have objections to the code assignment can apply for changes in accordance with the following provisions.

1. Certification materials

1. Application for change of Luo Health Code.

2. Extract the itinerary codes of medium and high-risk areas that you have not visited on your mobile phone and print them.

3. A negative nucleic acid certificate issued by a medical and health institution within 48 hours.

2. Nucleic acid sampling points for code-changing personnel

In order to facilitate nucleic acid testing for code-changing personnel and reduce the risk of epidemic spread at each sampling point, starting from the 7th, the First Affiliated Hospital of Henan University of Science and Technology will A new nucleic acid testing sampling point has been set up in the Luoyang Sports Center Velodrome, which mainly provides nucleic acid testing and sampling services for "yellow code" personnel who need nucleic acid testing to eliminate risks. The newly set up nucleic acid testing point is located at the intersection of Taikang Road and Xinyue Road in Luolong District. People who need testing can queue up through the west gate of the velodrome to enter the venue for nucleic acid testing. The testing point provides nucleic acid testing and sampling services from 9:00 to 17:00 every day, with no rest on weekends.

Personnel with "yellow code" who go to this point for nucleic acid testing need to provide their ID card, health code and "certificate" stamped with the official seal of the community where they currently live to take samples at the sampling point. The "Certificate" will be issued by family members or friends. For those who meet the transcoding conditions, the certificate will be issued by the community based on the person's actual situation.

When "yellow code" personnel go to nucleic acid testing sampling sites, they are not allowed to take public transportation. They must wear masks on site, maintain a "one-meter mask" and take personal protection measures, and pay attention to keeping warm from cold.

3. Change procedures

1. People in Xigong area who are in Luo are to be handled by family members or friends. Please submit the above certification materials to the leading agency for epidemic prevention and control in your neighborhood. The information officer responsible for the DingTalk system will change the health code. If you have any questions, please contact the sub-district office or community in your jurisdiction. 2. Family members or friends of people in the Xigong area who are not in Luo are required to send the above certification materials to the Municipal New Crown Pneumonia Epidemic Prevention and Control Headquarters Office by fax or email.
