The materials to be processed are: household registration book, birth certificate, vaccination list of hospital delivery room where the child was born, and health card.

Urban children should apply for children's vaccination certificate at the Center for Disease Control and Prevention in the area where their household registration is located. Rural children should apply for vaccination certificates in the planned vaccination clinics of township health centers under their jurisdiction.
