Medical insurance application process:

Medical insurance card: It is a special card for medical insurance personal accounts. Using the personal ID card as the identification code, the personal ID number, name, gender, and detailed information such as account fund allocation and consumption status are stored. 1. After receiving and filling in the form to participate in the insurance registration, the person in charge of the insured unit will submit the "Social Security (Citizen) Card Application Form" double-sided After copying, send it to each new insured person, and instruct the new insured person to fill in the application form in accordance with the requirements of the application rules.

2. The person in charge of the photography insurance participating unit will arrange for the new insured person to bring their ID card (or military officer’s ID card or passport) to the designated photography studio for social security (citizen) cards before the 3rd day of the month following the insurance registration. Take a digital photo of the social security (citizen) card and paste the photo receipt in the corresponding position on the application form. The fee for taking photos of social security (citizen) cards is 4 yuan/person.

3. To collect the application form and production fee on behalf of the insured unit, the manager of the insured unit shall submit the completed "Social Security (Citizen) Card Application" for the new insured person before the 19th of the month following the insurance registration. Collect all the forms and collect the social security (citizen) card fee of 20 yuan/person at the same time.

4. Submit the form, pay the fee and collect the card. The bank where the medical insurance account is opened will notify the insured policy location after the 19th of the month following the insurance registration. Go to the Guangzhou Medical Insurance Center to complete the formalities of submitting the form, paying the fee and collecting the card. The procedures for form submission, payment and card collection: can be handled at designated windows in the service hall of the medical insurance center.

5. The person in charge of the card-issuing insurance unit must issue the social security (citizen) card to the insured person within one week after receiving the card.
