③After confirming the loss report by phone, the settlement function of the card will be stopped through the medical insurance network within 24 hours.

④ After reporting the loss by phone, the person reporting the loss should complete the written loss reporting procedures within two working days. If the written loss report procedure is not completed within the specified time, the municipal medical insurance agency will restore the settlement function of the original social security card.

Note:

Before accepting the phone loss report and stopping the settlement of the card, the economic losses caused by the card will be borne by the individual. If you fail to report the loss in writing in time, the economic losses caused after the system automatically cancels the loss will also be borne by the individual.

B. Written loss report:

① After the medical banknote is lost, you should promptly go to the medical insurance agency with your ID card to complete the written loss report procedure.

② When reporting the loss in writing, the insured should fill in the "**City Urban Employees Social Security Card Loss Report Form".

③After accepting the written report of loss, the financial settlement function of the card will be stopped immediately through the Bank of China financial network, the medical insurance personal account will be frozen immediately through the social insurance network, and the settlement function of the card will be stopped within 24 hours.

Insured persons can go through the written loss reporting procedures at any urban medical insurance agency or Bank of China business outlet.

If entrusting another person to handle the matter, the entrusted person should present his or her ID card and register.

The person reporting the loss should promptly apply for a replacement card in accordance with relevant regulations.

D. Card replacement:

① After the insured person reports the loss of the medical coupon in writing, he or she can go through the card replacement procedure at the medical insurance agency with his or her ID card.

② When going through the card replacement procedures, the insured person fills out the "**City Urban Employees Social Security Card Reissue and Replacement Application Form".

③Five working days after applying for a replacement card, the insured person can go to the medical insurance agency to get the new card.

④ When the medical insurance agency accepts the application for card replacement, it will charge the cost of production and issue a receipt in accordance with regulations.

E. Unlock:

Note:

If the card replacement procedure is completed and the old card is retrieved during the card replacement period, the old card will be invalid.
