How to obtain the honorary certificate of voluntary blood donation in Zhejiang Province

2. Click "Certificate of Honor Application"

3. On the "Certificate of Honor Application" page,

Please read the "Application Notes" carefully.

Click "I agree".

4. Reminder: the city chooses "Jinhua".

Check whether the relevant information is consistent and proceed to the next step.

5. Follow the prompts.

Upload my second-generation ID card and one-inch red background photo,

When you are finished, click Next.

Warm reminder: if the uploaded photos do not meet the requirements, they will be returned and uploaded again, which will affect the authentication process!

6. After the blood donor information is uploaded and confirmed,

Note that the uploaded copy of ID card and one-inch photo with red background should be clear, otherwise the audit will not pass.
