On February 24, 2002, at 65438, a village in the northeast street of Yuyao City, the red bricks piled by Zhang suddenly collapsed, crushing the neighbor's low bungalow and crushing the 4-year-old boy sleeping in the room to death.

I. Overview and process of the accident

A few days ago, Zhang of a village in Northeast Street bought a lot of red bricks for building houses and piled a brick pile 6 meters long and 2 meters high on his uneven open space. Next to the brick pile is a low bungalow where Nitong Village lives. At noon that day 12, Ni's grandson fell asleep, and Ni and his wife were outside the door. When they suddenly heard a loud bang rushing back, their house was in ruins and their 4-year-old grandson was crushed to death on the spot.

There are three main reasons for the accident:

Case 3 Analysis and Thinking of an Electric Shock Accident

On July 10, 2000, an electric shock accident occurred in a chemical plant in Henan Province. 1 The worker stepped on the cable joint of the electric sewing machine, jumped up about 1m after being electrocuted, and fell heavily, but no fatal accident was caused after being rescued.

First, after the accident

On the morning of July 10, 2000, temporary farmer Han Mou (2 1 year old) and three other workers were engaged in the packaging of chemical products. 10, the monitor asked Han to get the plastic woven bag. When Han came back, he stepped on the cable on the ground and got an electric shock. Other workers at the scene hurriedly pulled off the cable and pulled down the knife. Second, the cause of the accident

After receiving the notice, the safety management personnel immediately rushed to the scene and protected the scene of the accident. The field investigation found that:

1. The cable length of sewing machine is about 20m, which is composed of three cables with different specifications, and the cable ends are not wrapped properly. Check the quality of cables, which are fake and shoddy products;

2. No electric shock protector was found at the scene of the accident;

3. At that time, due to the continuous rain, the chemical products had strong water absorption, the cable adhesive was wet, and the cloth shoes on Han's feet were soaked by water, so the grounding resistance of the cloth shoes was actually zero.

Third, thinking about the accident.

1. According to the above accident investigation, the workshop director and department staff were inquired, and the plan of replacing cables and configuring electric shock protectors was reported long ago, but the leader replied that he had no money, failed to rectify in time and take effective measures to implement monitoring, and the hidden danger of accidents turned into accidents.

2. Based on the analysis of the situation at that time, if a reliable electric shock protector is installed, the switch of the electric shock protector may not be closed at all under the condition that the cable is wet, and the accident is impossible. Even if you can barely close the switch and step on the line with wet feet, the action current of the electric shock protector will definitely exceed several times, and the power supply will be cut off.

3. After the accident, the workshop carried out serious rectification, updated cables, equipped with electric shock protectors, and scientifically arranged lines, and only invested 150 yuan.

Case 4 Guangdong "98.7. 13" mechanical injury accident

I. Overview and process of the accident

1July 3, 1998, Feng Mou and Lei Mou (female), the middle shift operators in charge of No.3 steel strip post in a concentrator in Guangdong, went to work on time on 15: 30. At 22: 55, Feng and Lei began to shovel ore and clean up the accumulated ore on the site and on the sidewalk. Feng shoveled the deposit on the sidewalk, about 18 meters away. When the accident happened, Feng was the only one at the head of No.3 steel core belt conveyor. Less deposits on non-sidewalks, more deposits on sidewalks. After feng shoveled the deposits on the non-sidewalk, he was ready to shovel the deposits on the sidewalk. For convenience, Feng took a shovel with a length of 1.8m and illegally crossed the No.3 steel core belt running at a speed of 2m per second, that is, the distance between the tensioner of No.3 steel core belt and the discontinued1No.3 belt was only 0.54m wide and 1.24m high. When Feng was flying in the air, he accidentally bumped into a shovel 1.8 meters long in his hand, dragging the shovel and people into the steel belt tightening wheel. The wooden handle of the spade was broken into three pieces, and Feng's head rested on the bracket outside the tightening wheel. Under the pressure of high-speed belt, Feng's head was cut off. 2. Feng's awareness of safety and self-protection is not strong. He violated the provisions of "No crossing the belt" and "Attention must be paid to the safety of equipment and people when cleaning the place where the equipment is running" in the Safety Operation Rules for Steel Core Belt Workers. He thinks he is very experienced and will not have any problems. Fortunately, he took a long-handled shovel and crossed the running steel core belt illegally, which seriously violated the operating rules, which was the main reason for the accident.

3. Lack of foresight in management, failure to strengthen protective facilities and improve the intrinsic safety level of equipment are the management reasons of this accident.

There is still a gap in safety education, which is another reason for accidents in management.
