What is the process of handling the disability certificate?

1. Application: Applicants who apply for a disabled person's card for the first time and those who apply for a second-generation disabled person's card for the first-generation disabled person's card must apply to the county-level disabled person's Federation where their household registration is located with their ID card, household registration book and six two-inch bareheaded photos;

2. accept. After receiving the relevant formalities submitted by the applicant, the county-level Disabled Persons' Federation shall check the applicant, photo, ID card and household registration. Input the relevant information in the application form into the basic database of the disabled population, and those who fill in false information will not be accepted;

3. evaluation. For applicants with obvious disability characteristics, the county-level Disabled Persons' Federation can directly fill in the evaluation form, and clearly record the disability characteristics and intuitive evaluation in the evaluation form, but it must be jointly evaluated and signed by three people, including the chairman.

4, political review, the county disabled persons' Federation chairman according to the applicant's relevant materials and hospitals or specialized medical institutions to make a preliminary assessment of disability. If the information is false or the disability assessment results made by medical institutions do not meet the disability standards, they shall be returned.

Procedures for issuing disability certificates.

(1) Application: An applicant who applies for a disabled person's card for the first time must present the applicant's resident ID card, household registration book and three recent two-inch bareheaded photos with a white background to the county-level Disabled Persons' Federation where his household registration is located, and truthfully fill in the application form and evaluation form. When applying for a certificate of mental and mental disability and a minor's application for a certificate of disability, you must also provide proof of the legal guardian. (2) Acceptance: After receiving the application materials submitted by the applicant, the county-level Disabled Persons' Federation will confirm the applicant, legal guardian, photo, ID card and household registration book, and those who fill in false information will not be accepted.

(3) Assessment: The designated institution will assess the disability of the applicant who applies for the Disabled Persons' Card, make a clear conclusion on the disability category and level according to the disability standard, fill in the assessment form and affix the official seal. If the assessment conclusion meets the disability standard, it shall be publicized in the village (community) where the applicant is located, and the publicity time shall be five working days; If the applicant is a minor, it shall not be publicized in principle.

(4) Examination and approval: The county-level Disabled Persons' Federation shall examine the application materials, acceptance procedures, disability assessment conclusions and publicity results, and complete the examination within ten working days.

If it meets the requirements after examination, it shall be approved, and the relevant information of the disabled card shall be filled in and printed, and the official seal shall be affixed to the column of the approving authority, and the seal shall be affixed to the portrait of the holder. At the same time, the relevant information such as the disability assessment form is entered into the basic database of the disabled population.

If the appraisal conclusion does not meet the disability standard, it shall not be handled.

(V) Issuance and archiving: The county-level Disabled Persons' Federation issues disabled persons' cards to the applicants, and archives the application forms, evaluation forms, publicity results and other related materials for long-term preservation.
