Relevant personnel can enter through the WeChat scan code or log in to the "Love Shandong" APP, and actively declare the origin, destination, nucleic acid detection, contact information and other related contents.

The "Notice" requires that all localities should widely post the two-dimensional code of "self-declaration of entry and exit Shandong personnel" at various traffic stations, highways and other exits, community public places, hotels, restaurants, shopping malls and other crowded places, so as to facilitate the scanning code declaration of entry and exit Shandong personnel.

The notice also made it clear that those who falsely report, fail to report, delay to report or fail to report the travel history, residence history and health status of key epidemic areas, forge and use false negative proof of nucleic acid testing, refuse to cooperate with nucleic acid testing and health monitoring, and bring risks to epidemic prevention and control will implement centralized isolation medical observation, and the resulting expenses will be taken care of by themselves, and the relevant personnel will be held accountable according to law.

All localities should widely post the QR code of "Entry-Exit Shandong Personnel Self-Declaration" in traffic stations, highways and other exits, community public places, hotels, shopping malls and other crowded places, so as to facilitate entry-exit Shandong personnel to scan the code on WeChat or log in to the "Love Shandong" APP for the first time, and actively declare the origin, destination, nucleic acid detection, contact information and other related contents. The system will set up "self-declaration terminal" and "follow-up registration terminal". The "self-declaration terminal" is mainly used for personal declaration of people entering and leaving Shandong, employment reception, declaration of hotels, restaurants, communities and other units. "Follow-up registration terminal" is mainly used for daily investigation and follow-up registration of follow-up personnel in grassroots society.
