First of all, you need to register MySpace and log in to the backstage of the website. MySpace has four product modules: contact, form, email and SMS. Click here to enter the "Form" module and start "Creating a Form". Next, create a questionnaire. At this point, you can create a blank form, add elements and contents yourself, or choose the appropriate template from the "Survey" module provided by Mike, and edit and modify your own contents. Then, after selecting the questionnaire template, click "Use this template" at the bottom right to enter the production process.

Finally, the questionnaire should be edited. On the left side of the page is the component area, which contains 20 basic components such as "selection, grading, sorting and matrix" and 30 contact components needed to make a questionnaire. In the middle is the questionnaire editing box. Hold down the selected component with the mouse and drag it to the specified location. Finally, edit and save the component content through the setting area on the right. According to the actual needs, you can set the overall appearance style of the questionnaire, add the question number, set the feedback arrival reminder, and only fill in WeChat. After completing the questionnaire, click "Publish" and the publishing page will automatically generate a QR code and a link. Save the generated QR code and share it.
