What are the symptoms of bladder tumor?
Symptoms of bladder tumor
1, hematuria
Hematuria is the most common initial symptom of bladder cancer, and 85% patients can have repeated painless intermittent gross hematuria. The amount of bleeding can be more or less, and there is a blood clot in severe cases. In the whole process of bladder cancer, 100% will have hematuria sooner or later.
2, bladder irritation symptoms
Infiltration of cancer itself, ulcer, necrosis, infection and congestion of cancer tissue can all be stimulating factors, which make bladder muscles contract and produce urination; Frequent urination, urgency, dysuria and urinary retention, persistent swelling and pain in the waist, urinary incontinence when cancer invades sphincter. Anyone with symptoms of bladder irritation is usually a sign of poor prognosis.
3. Dysuria
Cancer tissue falls off or the tumor itself and blood clots block the inner mouth of the bladder, causing about 7% dysuria and even urinary retention.
4. Symptoms of upper urinary tract obstruction
When cancer invades ureteral orifice, it will cause hydronephrosis, even infection of renal pelvis and ureteral orifice, causing backache and fever in different degrees. If bilateral ureteral orifice is invaded, symptoms of acute renal failure may occur.
5, lower abdominal mass
The first symptom accounts for about 3%, most of which are adenocarcinoma at the top of bladder or solid carcinoma of bladder with high malignancy in other parts. Rectum (or vagina) digital examination or solution and uneven mass are used to understand the scope and depth of tumor infiltration into bladder wall, which is helpful to estimate tumor staging.
6, systemic symptoms
Nausea, loss of appetite, fever, emaciation, anemia, weakness, cachexia, leukemia-like reaction, etc.
7. Metastatic symptoms
The tumor spread to pelvic cavity, retroperitoneal cavity or rectum, causing low back pain and lower abdominal pain to radiate to perineum or thigh and rectal irritation. Pelvic lymph node metastasis is more common, which transfers to uterus, rectum, colon, liver and kidney, causing corresponding clinical symptoms of various organs.
Treatment of bladder tumor
There are many treatments for bladder tumor. For each patient, what kind of treatment should be considered according to the tumor tissue type, case classification, general situation, patient's wishes, hospital conditions, doctor's skills and related social, cultural and economic background.
Due to the limited space, only the most common treatment methods are introduced.
Transurethral resection of bladder tumor (TURBt): This is an internationally recognized standard surgical method for the treatment of superficial bladder tumor. Superficial tumor refers to the tumor with clinical stage of Ta or T 1. This tumor is confined to mucosa and submucosa and does not invade the muscularis. Therefore, the probability of lymphatic and blood metastasis is extremely low, and the 5-year survival rate is over 80%, of which only 15% needs other more active treatment.
Transurethral resection of bladder is also suitable for invasive bladder cancer, even a 5cm tumor can be removed by transurethral resection. At the same time, the first transurethral resection of bladder tumor is the most reliable method to judge the infiltration depth of bladder tumor. However, the accuracy of staging is relative, and different pathologists will draw different staging conclusions for a specific pathology.
The probability of myometrial infiltration in well-differentiated tumors confined to the epithelial layer is not high, but the probability of myometrial infiltration in tumors with poor differentiation of intimal incision can be as high as 46%. Therefore, T 1 tumors, especially poorly differentiated Tis tumors, should be aware of the potential progress trend.
For T 1 stage tumors, it is common that the tumor is not completely removed during the operation, although the doctors who perform electrosurgery think that the tumor has been completely removed. In Germany, a research report pointed out that more than 40% of T2 tumors were resected 6 weeks after the first resection, and residual tumor tissue was found. The curative effect of preventive bladder perfusion after resection of bladder cancer also proves the existence of residual tumor after resection from another side.
Partial cystectomy: Partial cystectomy has a history of 100 years, and it was widely used before TURBt operation. The operation is simple and can preserve bladder function, which is easily accepted by patients.
Indications: Mainly superficial bladder cancer with large tumor size, wide tumor pedicle or difficulty in TURBt resection, locally invasive bladder cancer without distant metastasis, bladder diverticulum cancer and tumor belong to T2 and T3a invasive bladder cancer, but the patient refused to undergo total cystectomy.
Relative contraindications: carcinoma in situ, recurrent or multiple tumors, tumors exceeding the bladder wall, invading the bladder neck or prostate, too small bladder capacity after tumor resection (less than normal 1/3) and the patient's physical condition is extremely poor.
Can bladder tumor be cured?
The appearance of bladder cancer in life not only brings serious harm to people's health, but also brings great pressure to people's spirit. So many patients have a question: is this disease easy to treat? Because the effect of treatment determines the happiness of the family, we must carefully analyze the symptoms.
The most common early symptom of bladder cancer is hematuria visible to the naked eye without any feeling. This is a unique "abnormal urination signal" of bladder cancer, which will appear in almost every bladder cancer patient, so bladder cancer patients will see a doctor. There are two kinds of hematuria, one is gross hematuria and the other is microscopic hematuria. Gross hematuria refers to bloody urine that the eyes can look directly at, and microscopic hematuria refers to the presence of red blood cells in urine that can be found under a microscope. Hematuria of bladder cancer is mostly painless intermittent hematuria, mostly full-course hematuria, a few are terminal hematuria, and frequent micturition and urgent hematuria are rare.
Early and late symptoms of bladder cancer are different. In fact, all these contents tell patients to control it in time, and taking Tihengjian selenium Weikang chewable tablets in advance is helpful for prevention. Early interventional therapy can greatly improve the cure rate of the disease and avoid a lot of pain, but in the later stage, it may not have a good effect anyway, so we must seize the golden treatment period.
