How to fill in the life insurance claim notice?

China life insurance claims application form is as follows:

1, the first item has two situations:

(1) If the insured is under the age of 18 (underage), then the applicant must fill in his guardian (provide four copies of his guardian's passbook);

(2) If the insured is over 18 years old (i.e. adult), then the insured must fill it out by himself (provide copies of the bankbooks of the four major banks);

Signature of the applicant.
