I. Identification category:

Linxia City Education Bureau is responsible for identifying the qualifications of senior middle school teachers, secondary vocational school teachers and secondary vocational school internship instructors; County Education Bureau is responsible for the qualification of junior high school teachers, primary school teachers and kindergarten teachers.

Second, the applicant shall abide by the Constitution and laws and regulations of People's Republic of China (PRC), support the leadership of the * * * production party of China, implement the party's education policy, adhere to the socialist orientation of running schools, love education, fulfill the obligations stipulated in the Teachers Law, and abide by the ten criteria of teachers' professional ethics and teachers' professional behavior in the new period. Strict ideological and political and professional ethics inspection, there are violations of teachers' professional ethics and the ten standards of teachers' professional behavior in the new period, and those with criminal records will not accept the application for teacher qualification for the time being. Third, the scope of identification:

1. China citizens who have not reached the national statutory retirement age and whose household registration and residence (must hold a local residence permit and be within the validity period) are in Linxia Prefecture;

2. Soldiers and armed police stationed in Linxia Prefecture;

3. Hong Kong, Macao and Taiwan residents who hold the residence permit of Hong Kong, Macao and Taiwan residents within the validity period of our state can apply for the qualification examination of primary and secondary school teachers in our state and identify the qualifications of primary and secondary school teachers in our state; Hong Kong and Macao residents who hold Hong Kong and Macao residents' passes to and from the Mainland and residents of Taiwan Province Province who are valid for five years can apply for the qualification examination for primary and secondary school teachers in our province, and the qualification of primary and secondary school teachers is recognized at the place where the examination is held. Other conditions, procedures and materials submitted by Hong Kong, Macao and Taiwan residents applying for teacher qualification in our state are the same as those submitted by our state applicants;

4. The applicant shall pass the national primary and secondary school teacher qualification examination, and have a certificate of qualification examination for primary and secondary school teachers issued by the Education Examinations Institute of the Ministry of Education (within the validity period). To apply for the qualification of primary and secondary school teachers as teaching departments and disciplines, it shall be consistent with the qualification certificate of primary and secondary school teachers;

5. Postgraduates and normal students who meet the qualification reform of teachers in primary and secondary schools without examination shall obtain the qualifications of teachers in the corresponding periods and disciplines with the "Teachers' Professional Ability Certificate" within the validity period. Those who apply for the qualifications of teachers in other subjects and disciplines are not exempt from the examination, and must take the national primary and secondary school teacher qualification examination.

Four, that conditions:

1. According to the provisions of the Teachers Law, those who have obtained the qualifications of teachers in primary and secondary schools (kindergartens) shall have the following corresponding academic qualifications.

To apply for the qualification of kindergarten teachers, one should have a diploma in pre-school education with secondary education or above offered by kindergarten normal schools or other institutions with school qualifications. (List of pre-school qualified schools with secondary education level in Gansu Province (see annex))

To apply for the qualification of a primary school teacher, one should have graduated from a secondary normal school or above.

To apply for the qualification of a junior high school teacher, one should have graduated from a teacher's college or other college or above.

To apply for the qualification of a senior middle school teacher, one should have a bachelor's degree or above from a normal university or other university.

To apply for the qualification of a teacher in a secondary vocational school, one should have a bachelor's degree or above from a normal university or other university.

To apply for the qualification of internship instructor in secondary vocational schools, one should have graduated from secondary vocational schools or above, and have a professional and technical position of assistant engineer or above or a technical level of intermediate worker or above.

2. Education and teaching ability conditions.

Participate in the national primary and secondary school teacher qualification examination, pass the written examination and interview, and receive and hold the certificate of primary and secondary school teacher qualification examination issued by the examination center of the Ministry of Education, and it is within the validity period.

3. Putonghua proficiency certificate: The applicant's Putonghua proficiency should reach the second-class B or above standard of Putonghua Proficiency Test Grade Standard issued by the State Language Committee. Among them, Chinese teachers and teachers of Chinese as a foreign language should reach the second-class A and above standards; Phonetic teachers and professional teachers such as broadcasting, hosting, film and television drama performance should reach the first-class and above standards; To apply for kindergarten teacher qualification, you should reach the second-class and above standards. ? 4 should have good physical and psychological quality, can adapt to the needs of education and teaching. No infectious diseases, no history of mental illness, and passed the physical examination in public general hospitals (non-designated hospitals) at or above the county level (the results of physical examination must be accompanied by liver function test sheets). Applicants during pregnancy can be exempted from physical examination items that are not suitable for pregnant women. If other detectable items are qualified, they shall be deemed as qualified in the physical examination, but the chief physician must sign the physical examination form to explain the pregnancy. When submitting the physical examination certificate, the applicant should attach the test report of positive pregnancy reaction or the file of perinatal examination. Five, to apply for the qualification of primary and secondary school teachers on-site confirmation should produce the following application materials:

1. Original second-generation ID card of the applicant (within the validity period);

2. Relevant household registration certification materials: if applying for household registration, submit the original household registration book or collective certificate; To apply for residence identity certificate, submit the original residence permit within the validity period; Where the school is located, the original student ID card with complete registration information shall be submitted; Hong Kong, Macao and Taiwan residents apply for recognition, and submit the original mainland travel permit for taiwan residents with the validity period of five years, such as the residence permit for Hong Kong, Macao and Taiwan residents or the pass for Hong Kong and Macao residents to and from the mainland; Servicemen and armed police stationed in Linxia Prefecture shall provide personnel relationship certificates and valid serviceman (armed police) certificates issued by their subordinate units or personnel departments;

3. The original "Physical Examination Form for Applicants for Teacher Qualification in Gansu Province" (valid at that time, see the attachment for the template);

5. To apply for the qualification of internship instructor in secondary vocational schools, you must provide the original certificate of professional and technical position equivalent to assistant engineer and above or the certificate of technical grade of intermediate worker and above;

6 Hong Kong, Macao and Taiwan residents applying for the qualification of primary and secondary school teachers shall submit the certificate of no criminal record issued by the relevant departments of Hong Kong Special Administrative Region, Macao Special Administrative Region and Taiwan Province Province. If necessary, the applicant can apply to the Gansu Provincial Teacher Qualification Guidance Center through the accreditation body in advance to issue relevant letters.

7. Other supplementary materials. (The following materials will be compared through the national identity information system when the applicant declares online. If the comparison is successful, it does not need to be provided on site; If the comparison is unsuccessful, it must be provided on the spot. )

(1) Original academic certificate. 8. Paper materials that do not need to be presented when applying for on-site confirmation of primary and secondary school teachers' qualifications: the certificate of teacher qualification examination and the certificate of teachers' professional ability of education graduate students and public-funded normal students are compared through the information system of China Teachers' Qualification Network, and need not be presented on site.

5. The information provided by the applicant on the Internet must be true and effective. If the information provided is untrue and inaccurate, and the teacher qualification cannot be confirmed, the responsibility shall be borne by the applicant; Applicants can only apply for one teacher qualification certificate at a time, and they cannot apply for a second teacher qualification certificate nationwide within one natural year after successful application.

6. The applicant shall truthfully fill in the personal letter and be responsible for its contents. If it is inconsistent with the facts, the education administrative departments at all levels will, in accordance with the relevant laws and regulations of the state, impose administrative penalties on the applicant, such as "revoking the teacher's qualification and not re-obtaining the teacher's qualification within five years" or "losing the teacher's qualification and not re-applying for the teacher's qualification".
