Medical insurance card cancellation process

1. After completing all medical insurance reimbursement business, take a copy of death certificate to Kunming Sub-center of Social Insurance Fund Management Center to issue the Notice of Loss Reporting and Settlement of Medical Insurance Card.

2. With the Notice of Reporting the Loss of Medical Insurance Card and related documents, cancel the account at the business card printing center of Agricultural Bank of China.

To cancel the medical insurance card and social security card, you need to provide materials: I or my agent bring the original and photocopy of the insured's ID card, and if you handle it as an agent, you need to provide the agent's valid certificate and photocopy, the social security card to be cancelled, and the certificate of termination of the business related to human resources and social security.

If it is necessary to cancel the social security card, please ask the insured person or agent to go to the human resources and social security business window to handle various businesses and related termination procedures, bring relevant certificates issued by human resources and social security to the card center window to cancel the social security function, and then go to the designated bank window to cancel the banking function and return the social security card.
