Notice on the physical examination for Shandong College Entrance Examination Admissions in 2019 (2)

Behavior.

1. The conclusion of the physical examination must be accurate, and the measurement data of vision, height, weight, blood pressure, etc. must be accurate and try to achieve zero error to prevent malicious errors. The data filling in the "Candidate Physical Examination Card" must comply with the standards. The written descriptions of abnormal physical examination items must be neat and clear, and the content must be accurate to avoid ambiguity due to excessive omissions or illegible writing. Records in the "Candidate Physical Examination Card" are generally not allowed to be altered. If it is really necessary to make alterations, the alterations should be signed by the medical examiner himself and stamped with a special seal for alteration, otherwise they will be invalid. After the physical examination is completed, the signature of the examining doctor is required.

2. The "Candidate Physical Examination Card" must be sent, received, delivered and kept by a designated person. Candidates and non-physical examination staff are not allowed to touch the card.

3. For difficult cases, consultation and review are required and no hasty conclusions are allowed.

4. In the final stage of the physical examination, it is necessary to organize personnel on a county (city, district) basis to review the "Candidate Physical Examination Card" of all examined candidates to check whether there are any missed examinations, missing fillings, messy writing, unclear handwriting or incomplete writing. Provide seals and other information, and make timely corrections if problems are discovered.

5. Candidates should be notified of the results of the physical examination in a timely manner. For medical examination conclusions that affect normal admissions (which may cause the university to deny admission), a dedicated person should be arranged to inform the candidates in person, so that they can fully understand the possible consequences of the conclusion, as well as the appeal procedures and re-examination regulations.

6. Do a good job of re-inspection. If candidates have objections to the conclusion of the physical examination, they should apply to the county (city, district) admissions examination agency within one week after the physical examination. After approval by the physical examination leading group at the same level, a reexamination can be carried out. If the candidate still has objections to the reexamination results, the hospital designated by the Provincial Admissions and Examination Committee will ultimately make an arbitration conclusion on the objections. The re-inspection and arbitration work must be completed before April 25.

7. Unless medically necessary, fully nude examinations are generally not recommended.

8. After the "Candidate Physical Examination Card" is reviewed and found to be correct, it will be summarized by the municipal admissions and examination agency and reported to the Information Management Office of our hospital within the specified time (the specific submission location will be notified separately).

9. The conclusion of the physical examination or reexamination conducted at the specified time shall prevail. If it is not conducted within the specified time, the conclusion will not be used as the basis for admission.
