Model application for collective account transfer? Now I am an employee of the company, with my ID number. Because I do not have the conditions to settle down, I now apply to move my account into your collective household and agree to the following terms:

1. I promise to abide by the national laws and regulations on family planning management and ensure that I will not violate the national laws and regulations. Otherwise, I will bear all the consequences, and take the initiative to move my account out of the Cizhi collective household and confirm that Cizhi will no longer handle all matters related to my collective household.

2. Accept and abide by your company's regulations on the management of collective households during the period when the registered permanent residence moves into your company's collective households, and actively assist and cooperate with your company to do relevant work.

I promise that I will move my account out of your collective household in time when I have the conditions to settle down.

4. After leaving my job, if the company has any objection when my account moves out, I am responsible for communication and consultation with the company. At the same time, I agree that my account will continue to be managed by your company before the above matters are properly handled, and I will temporarily move out and pay the account management fee according to the standard of 50 yuan every month.

5. If our company fails to fulfill the personnel agency contract (or agreement) signed with your company, and I owe your account management fee during this period, I need to use my account card, and I will ask our company to make up the owed' management fee' or pay it myself. Otherwise, your company has the right to refuse to provide account cards and related services.

6. After my labor relationship with your company is dissolved or terminated, or after my personnel agency contract (or agreement) with your company is dissolved or terminated (in the middle), I promise to move my account out of your company's collective account in time in the month of dissolution or termination (in the middle). If I fail to move out, I will be deemed to automatically accept and abide by your account management regulations. From the month following dissolution or termination (mid-term), I will pay your company an account management service fee of RMB 50 yuan/month on a monthly basis until my account moves out.

7. After I dissolve or terminate the labor relationship with your company, or after I dissolve or terminate the employment introduction contract (or agreement) with your company, if I fail to pay or owe the management service fee, your company has the right not to handle matters related to my account, and I will unconditionally move out of my account in the month when I pay the management fee.
