After changing the suffix to m4r, you need to double-click the renamed ringtone file and add it to the ringtone in the database.

Check: In the itunes menu bar "Edit", find "Preferences" to open it.

Check "Ringtone" in "Common"-"Source", and then check "OK"

You can see the "ringtone" in the "database"

Turn on the "ringtone" and you can see if your file has been turned into a ringtone.

Then open "iphone" in Devices.

Select Ringtones, check the item "Synchronize Ringtones", select "All Ringtones" or "Selected Ringtones" and then select "Synchronize".
