Brief description of the accident:

Woodworking team leader Wei xx led Shao xx (a temporary worker) to the No.6 cooling tower to install the shaft gate. After the gate was lowered about 7mm along the gate slot, it couldn't be put down any more. Shao xx went on to look at it, opened a cover plate to face the shaft, and supported the two walls of the main tank with both hands to jump down. Just listening to the sound of "Ah", Shao xx (male, 25 years old, carpentry level 4, working in this type of work for five years) fell into the central shaft and slipped into the bottom of the circulating water pipe, with a drop of 11.33 meters, and died after being rescued.

Analysis of the cause of the accident:

Shao xx was too confident. He didn't step on the mouth of the distribution tank as a middle step and went down to the bottom plate of the main tank. Instead, he jumped down quickly and slipped into the central shaft when his center of gravity was unstable and his feet were suspended.

Precautions:

(2) Conduct safety technology education to improve the quality and awareness of employees.

2. Smoking in violation of regulations caused death

Main reason: violation of operation regulations

Casualties: one person died

Brief description of the accident:

Corrosion protection of the wall of No.6 water tower in Baotou No.2 Power Plant. Brushing is to use a slow hoist to lift a 6-meter-long hanging basket, upload five people, 2 barrels of paint and half a barrel of spare thin rice. When brushing from top to bottom to 6 meters, temporary worker Li xx and Huang xx smoked, throwing unburned matchsticks into the paint bucket, and the paint and thinners caught fire instantly, and the fire spread. Li xx (male, 25 years old, electrician, working in this type of work for three years and 1 months) was surrounded by a fire, and the fire was fierce. When the hanging basket landed on the ground, Li xx had been burned to death.

Cause analysis of the accident:

(1) The main reason for the accident is that the responsible persons at all levels in the construction site failed to take concrete and effective measures for fire safety.

(2) Li xx and Huang xx smoke next to flammable materials and throw unburned matchsticks into a paint bucket to catch fire. Violation of regulations is the direct cause of the accident.

Precautionary measures:

The batching and oil painting personnel are forbidden to smoke when working, and check whether they carry kindling or not before taking up their posts, so as to ensure that fireworks are strictly prohibited in inflammable and explosive places.

3. The hanging tray is unprotected and the person falls

Main reason: lack of protective devices

Brief description of the accident:

Erector Guo xx and other three people led the accident. When the lifting table was lifted from 196 meters to 197.5 meters, two of the six inverted chains had already reached the full stroke. At this time, Guo xx and Guo xx each changed a chain. When the chain hook is removed, it causes the hanging plate to shake slightly. At this time, Yang xx, a migrant worker, stood on the higher side of the hanging tray (the tray diameter is 5.4m due to the unsynchronized pulling of six migrant workers), which is only about 2～3mm away from the edge of the hanging tray. At this time, there was a gap of 4mm between the hanging plate and the brick lining, and Yang xx (male, 19 years old, the contractor) fell to the ground of meters from this gap and died after being rescued.

Cause analysis of the accident:

(1) The lack of protective measures for the hanging plate and the failure of the construction personnel to wear safety belts are the important reasons for the accident.

(2) There was no unified command by a special person when lifting the lifting table, and the six inverted chains were not synchronized, which caused the lifting table to tilt, resulting in a large gap on one side of the lifting table. The direct cause of the accident was that people stood unsteadily and fell because of shaking.

Preventive measures:

(1) Improve the safety protection facilities of the hanging tray, and fasten the safety belt when working at heights.

(2) The lifting of the hanging plate should be commanded by designated personnel to prevent the hanging plate from tilting.

4. One person sawed the long material and stabbed the wood into his left eye and died

Brief story of the accident:

Sun xx used a chainsaw window wood to change a 4× 6× 2,59 mm piece of wood into a 3×4 mm square. When the square pushed into the kerf for 94 mm, and there was still 65 mm left, the square was bounced off by the saw blade.

Analysis of the cause of the accident:

When sawing a square wood of 4× 6× 2,59 mm, one person should push the material and one person should receive it, and one person should operate it, which caused the sawn material to be bounced by the saw blade, which broke the square wood and caused an accident.

Precautions:

(1) When sawing long materials with a sawing machine, it should be operated by two people.

(2) Before sawing, the edge tip should be cut off.

(3) Strictly abide by the operating rules of the sawing machine and wear personal protective equipment.

5. Ignorant to deliver small materials by hand, right thumb was sawed off

Date of accident: 14: on October 22nd, 197.

accident site: Baoshan power plant project construction site.

main reason: violation of operating rules.

Casualties: one person was seriously injured.

Brief description of the accident:

A carpenter (temporary worker) Yan xx was making a nail box in the carpentry room, but there was still a small piece of wood board (5×5 mm) missing, so he took an old wood board with a small amount of cement and sawed it off on the circular saw according to the required specifications. After that, Yan xx tried to cut off a small piece of 5 mm on the circular saw machine because the saw blade was too blunt, so he pushed it with both hands at the beginning. When the tail was nearly 7 mm, he used his right thumb and forefinger to press the wooden strip forward. As a result, the right thumb slipped into the saw slot and was cut off for a section and a half, causing serious injuries.

Analysis of the cause of the accident:

(1) The direct cause of the accident is that after entering the factory strictly, the operator has not been educated in the operation procedures of using the circular saw machine, and the small materials are delivered directly by hand, and the illegal operation.

(2) The circular saw machine and other woodworking machines in the woodworking room lack strict management, and the safety operation procedures are not carefully implemented, and poor management is one of the reasons for the accident.

Preventive measures:

(1) Implement safety operation procedures. When sawing small materials with circular saw, it is forbidden to feed directly by hand.

(2) Strengthen the management of woodworking machines and tools.

6. Three-finger injury caused by manual withdrawal of sawn materials with wet short materials and knot scars

Brief description of the accident:

Zhang xx, a fifth-class carpenter, used a circular saw to break a square wood of 6× 5× 1mm. Zhang's one-man operation was because the wood was wet and had a big scar, so the wood was sawed, and the square wood flew out. Zhang (male, 29 years old, a fifth-class carpenter, this kind of work lasted for 14 years) lost his balance, his right hand touched the saw blade, and his three fingers were sawed.

Analysis of the cause of the accident:

Zhang xx violated the operating rules, did not use the pusher, and there was something wrong with the material selection, resulting in serious injury to his right hand.

Preventive measures:

(1) Strictly implement the safety operation procedures of circular saws.

(2) Don't use damp and knotted wood.
