How to treat lymphadenopathy with traditional Chinese medicine

Lymph node enlargement is a common clinical sign, which can be local enlargement, systemic enlargement, or local enlargement first, and then develop into systemic enlargement. It can be caused by malignant hematological diseases and many non-hematological diseases. Malignant tumor lymphadenopathy is more common in malignant lymphoma, acute lymphoblastic leukemia, monoclonal immunoglobulin diseases such as multiple myeloma and macroglobulinemia. This syndrome belongs to the category of "scrofula" and "phlegm core" in TCM.

The etiology and pathogenesis of lymphadenopathy are mostly exogenous pathogenic factors, emotional internal injuries, improper diet, physical factors, etc., which lead to phlegm-dampness stagnation, cold coagulation of blood vessels and internal resistance of blood stasis, thus forming phlegm nuclei and scrofula. Among them, qi stagnation, phlegm turbidity, heat toxin or evil toxin intertwined, blood stasis, qi and blood obstruction, phlegm and blood stasis are the key pathogenesis of this disease.

The diagnosis of this syndrome requires detailed inquiry about medical history, lymphadenopathy and its accompanying symptoms, physical examination, laboratory examination, chest X-ray and abdominal B-ultrasound examination.

According to the clinical symptoms, Chinese medicine treats according to syndrome differentiation;

(1) Excessive phlegm-dampness: There are multiple swollen nuclei in the neck, under the ear or under the armpit, and under the skin of the mouse, which are neither painful nor itchy. The skin color is as hard as stone, and it can't be pushed away. The face is pale, fatigue, pale tongue with thin white fur and weak pulse. This syndrome can be treated with Yanghe decoction.

(2) Qi stagnation and phlegm obstruction: there are multiple swollen nuclei in the neck, armpit and rat skin, which are painless and itchy, with normal skin color, firm pressing, chills and fever, bitter mouth and dry throat, dizziness and tinnitus, anorexia and irritability, yellow urine, reddish tongue coating and rapid pulse. This syndrome can be treated with Chaihu Shugan Powder.

(3) Phlegm-fire accumulation: there are multiple swollen nuclei in the neck and armpit, and the skin color is normal, so it is firm, cold and hot, bitter mouth and dry throat, upset and irritable, easy to urinate, red tongue with yellow fur and slippery pulse. This syndrome can be treated with Daotan decoction.

(4) Blood stasis and phlegm coagulation: emaciation and abdominal distension, lump in the neck and axils, abdominal pain, nausea and vomiting, hot flashes in the afternoon, dry or black stool, dark tongue or ecchymosis, and heavy or astringent pulse. This syndrome can be modified by Biejia Wan Jian.

Treatment.

(5) Yin deficiency of liver and kidney: dizziness, hypochondriac pain, tinnitus, hard throat, dry mouth and throat, upset and fever, soreness of waist and knees, nocturnal emission or irregular menstruation, red tongue with little fur and rapid pulse. This syndrome can be treated with Qijudihuang Decoction.

(6) Deficiency of both qi and blood: dizziness, palpitation and insomnia, listlessness, pallor, numerous sputum nuclei under the neck and armpit, hard as stone, incessant pushing, emaciation, pale lips, chills, pale tongue, thin and white fur and thready pulse. This syndrome can be treated with Bazhen Decoction.

It can also be treated by external therapy and Chinese patent medicines such as Xiaojin Dan.
