Experience of pediatric continuing education
On weekdays, it is inevitable that there will be some new ideas in my heart. Writing an experience and recording it can help us sum up our previous thoughts, work and study. But what are the requirements of experience? The following is my experience in pediatric continuing education for your reference, hoping to help friends in need.
Pediatric further education experience 1 1 year has just ended, and I have returned to my familiar post. First of all, I want to thank the hospital and the leaders for giving me this precious opportunity, and I also want to thank my colleagues, because their hard work allows me to finish my studies with peace of mind.
I studied in the Department of Pediatrics of Xinhua Hospital affiliated to Shanghai Jiaotong University School of Medicine, which was founded in 1958. It is a modern comprehensive teaching hospital with complete disciplines and distinctive professional characteristics, integrating medicine, teaching and research. It has been awarded the title of city civilized unit for seven times, national advanced collective for three times, third-class first-class hospital 1993, national advanced hospital cultural construction unit, and Shanghai learning unit in 20xx. The overall strength of pediatrics in Xinhua Hospital is in the leading position in China and advanced in the world, and it has several national, provincial and ministerial key disciplines and key laboratories. Neonatal Department of Xinhua Hospital is now the key discipline of "2 1 1 project" of the Ministry of Education. For many years, he has been a member of the Neonatal Group of the Pediatric Society of Chinese Medical Association and the National Perinatal Society of Chinese Medical Association, and is a key discipline for the all-round development of clinical medical treatment, teaching and scientific research in hospitals.
The pediatrics department of Xinhua Hospital has 1 1 departments. Because of time, I mainly study in several important departments, namely: Children Intensive Care Unit (PICU), Neonatal Department (neonatal ward and NICU room), Nephrology Department, Respiratory Department, Neurology Department, Digestion Department, Hematology Department and Cardiology Department; In the first month, according to the roster, I was old and worked long hours, so I was placed in the children's intensive care unit. Because of my weak theoretical foundation, I know little about new diagnosis and treatment techniques and routine treatment procedures in hospitals. Besides, Shanghai dialect is not suitable for communication, and I am not used to eating. Especially in the night shift, I work almost all night, and sometimes it is difficult to complete the night shift task. The fast-paced life is unbearable, and I once had the idea of backing out. Later through my family and my family. Especially in the night shift, in the dead of night, I thought to myself, this is also an exercise in my career and an opportunity to improve my medical level. I must seize this opportunity and stick to the training plan, and I can't give up halfway. From then on, during my study, regardless of my age, I humbly asked my younger generation for advice. Although I am older than them, they are all my knowledge teachers. After a month of unremitting efforts, I not only enriched my professional knowledge, but also learned more updated and advanced technologies, and gradually adapted to the environment and fast-paced work here. I feel the joy of learning in my busy life, and I feel it in my busy life. Through learning, I deeply feel that the gap between our first-line hospitals is not only the technical level and advanced medical equipment, but also the work attitude and professionalism. I feel that the teachers here are noble in medical ethics, knowledgeable and approachable. Not only is the level of diagnosis and treatment among the best in China, but they also attach great importance to the cultivation of the younger generation and maintain a strong talent echelon.
Moreover, each post is specialized. Their department directors make rounds twice a week, and the second line makes rounds five times a week. I feel a harmonious atmosphere during rounds, emphasizing seeking truth from facts and attaching importance to evidence-based medical evidence. Many issues of principle have been recognized, but differences that do not violate principles are allowed. Xinhua Hospital has always emphasized comprehensive treatment, and the comprehensive department organizes joint rounds of pathology, surgery, internal medicine and obstetrics and gynecology from time to time. When making rounds, the first-line or senior doctors report the medical history; The superior doctor carefully reads the film, analyzes the possible diagnosis, further examination and treatment plan. In this way, patients can get reasonable treatment systematically and systematically, and young doctors know a lot about this. Reasonable comprehensive treatment is one of the important factors of high-level diagnosis and treatment in Xinhua Hospital.
Experience of continuing education in pediatrics. Good times always make people feel so short. 10 months of study life is coming to an end, and I am always reluctant to give up the humanistic care and knowledge hall of the general hospital.
Counting these short 10 months, from strangeness to familiarity, is the silent care of every leader, teacher, brother and sister in pediatrics department of general hospital, which makes me feel the warmth of home gradually in this strange land. The amiable and smiling director XXX still cares about every staff member in pediatrics during his busy schedule, covering everything from life to study, fully demonstrating the demeanor of parents. There are also deputy director XXX, deputy director XX, teacher XX, teacher XX and head nurse XX who give people meticulous care in life.
Because of the knowledge hall of pediatrics in general hospital, I feel that time is very short. The grandeur of the knowledge system always makes you feel that you can't finish learning and don't work hard. In the Department of Neurology, under the leadership of Director XX, the diagnosis and treatment of children's epilepsy, stroke, central nervous system infection, hereditary metabolic diseases and children's psychological and behavioral disorders are in the forefront of the country, and the national norms for the diagnosis and treatment of children's stroke have been formulated, and the basic and clinical research of infant spasms is in the forefront of the world. In the field of hematological tumors, under the leadership of Deputy Director XXX, the diagnosis, treatment and research of childhood leukemia, lymphoma, solid tumor (especially neuroblastoma) and aplastic anemia are at the forefront of the country, especially hematopoietic stem cell transplantation, which has brought good news and gained new life for many unfortunate children. In the general hospital, there is also a special team: NICU, a fragile little life who came to this world ahead of time and thrived under the careful care of the team led by Director XX.
The most fascinating thing is the learning atmosphere in the general hospital. Small to department: associate professors give lectures every Tuesday, departments meet on Thursday, and general difficult cases are discussed on Friday; There is also a convenient and quick document retrieval system in the department, which allows you to have a look at the latest developments of Chinese and foreign medicine. As big as a hospital: there are various academic activities almost every day, including an expert lecture hall that can't be shaken every Friday, and the most famous experts and scholars at home and abroad are invited to participate in international academic activities of various majors on weekends. Listening to the master's explanation is always refreshing, like a spring breeze.
10 months, fleeting, always feel that the time to live and study with you is so short, I really hope to have another chance to study in pediatrics of a general hospital.
Finally, I sincerely thank you for your concern and help. May the pediatrics department of General Hospital become stronger under the leadership of Director Zou, bringing benefits to more children and their families.
How time flies! In a blink of an eye, I am about to finish my further study in pediatrics. I have learned a lot during my three-month further study in pediatrics. I haven't done some work before and don't know the nature of pediatrics. Like an intern, I humbly ask the teacher for advice and study and practice by myself. Under the patient guidance of the teacher and the education of the head nurse, I have basically mastered the nursing of some common diseases in pediatrics and some basic operations, such as venipuncture, sputum aspiration, atomization and drug penetration. In a short time, I became familiar with pediatrics and made clear the process and direction of work. In the days of further study, I have constantly summed up my experience from the practice of pediatric nursing, summed up my three-month study, and shared the following experiences with you:
First, the particularity of patients.
The nursing object of pediatrics is children aged 0- 14. Most of them are babies. Children are afraid of nurses and it is not easy to communicate or cooperate, which requires us not only to have a sense of responsibility, but also to have love and patience. Another feature is too many companions. At present, most children are only children. Once sick, parents are particularly nervous and anxious, which puts forward higher requirements for the operation of medical staff. Therefore, communication with parents also needs special attention from pediatric nurses.
Second, the particularity of disease observation
Pediatric children's diseases are characterized by acute onset and rapid change. In addition, the child is too young to describe his illness. Therefore, nurses are required to be extra careful and patrol frequently when observing the condition, and at the same time, they should patiently educate their parents, communicate in time, find out the changes in the condition in time and deal with them in time.
Third, the particularity of nursing operation
Pediatric children's dosage is small, infusion steps are many, and medication is frequent. Nurses are required to calculate and dispense drugs accurately, and the operation should be gentle, accurate, fast and kind.
Fourthly, the particularity of venipuncture.
Pediatrics needs excellent venipuncture technique, especially scalp vein. It is very important to choose the main pulse. The vein of infusion should be thoroughly skinned and the angle of needle insertion should be small. When returning blood, it should be fixed immediately, and no more needles can be inserted.
Verb (abbreviation of verb) Suggestions on developing pediatric work
1. Establish a people-oriented service concept: "patient-centered" is not a slogan, but a manifestation of medical staff's respect for patients' humanity in medical activities. Without good professional ethics and skilled nursing skills, it is difficult to achieve patient, meticulous and thoughtful nursing work.
2. Excellent venipuncture technology and effective communication: venipuncture in children is difficult and parents have higher requirements. I hope that nurses can hit the nail on the head, strengthen the study of nurses' humanized nursing knowledge and strengthen the communication methods and skills between nurses and patients during venipuncture for children. Appellations should be affectionate, actions should be gentle, words should be gentle and considerate, and care and love for children should be reflected in all kinds of subtle actions.
3. Explain in place when doing nursing treatment and strengthen education: give children various treatments, such as explaining to children and parents how many drugs to use today, and explaining the usage of these drugs when adding drugs; It is necessary to explain the purpose and function of atomization, let children participate in their own disease treatment and actively cooperate with doctors. It is necessary to communicate with children and their parents in time and ask about their illness, so that children and their parents can always feel the concern of medical staff. Strengthen education to let family members know the symptoms, nursing and precautions of common diseases. Most new parents lack parenting knowledge and often love but don't know, especially when their children are ill and in urgent need of help. You can use bad publicity and blackboard newspaper to carry out knowledge education.
4. Improve the environment: according to the children's psychological characteristics, draw some cartoon patterns on the walls of the ward, replace the children's beds with colorful sheets and quilts with animal patterns, put a TV in the room and play their favorite programs, so that they can receive treatment in a relaxed and happy environment.
Pediatric learning experience 4 time flies. In a blink of an eye, my half-year study and study life in the neonatal department of Shenyang Shengjing Hospital has come to a successful conclusion. In the past six months, I seem to have returned to my student days. I managed patients with teachers in the department during the day and went back to the dormitory to check the information and recite the main points of knowledge at night. After half a year's further study, under the careful guidance of teachers, I have learned a lot, and my professional level and communication skills have been qualitatively improved.
First, systematically learn new knowledge, new technology and management system.
Through intense and extensive study, I have mastered many professional operations in neonatology, mastered the use of CPAP machine and ventilator, and further understood the professional knowledge of newborns, such as the treatment of very low and ultra-low birth weight infants, the treatment of pneumothorax, the prevention and treatment of necrotizing enterocolitis in premature infants, the treatment and reading of respiratory distress syndrome and pulmonary hemorrhage in premature infants, the treatment of pulmonary hypertension patients, the management of ventilator patients, and the observation and treatment of infections in newborns and premature infants. After work, departments often organize some business studies to continuously improve their own literacy.
Second, feel harmonious teamwork and professional attitude.
The first is good teamwork. The department has five wards, which are relatively independent and unified. Each colleague cooperates with each other while completing his own work to ensure the management quality of the ward. I am very touched by the professionalism shown by the director and doctors here. They teach patiently and meticulously and take pains to answer questions. They will answer my questions, comments and suggestions in time. Give me a lecture as soon as you are free, and take pains until you understand it.
Third, experience warm and meticulous humanized service.
When I first came to the hospital, my first impression was neat, clean and warm. What impressed me most was that they didn't pay lip service to humanized service, but really cared about children. For premature infants, the environment is required to be relatively quiet, the sound and operation should be as light as possible, and the stimulation of sound should be reduced. All treatments are centralized, and there will be a cartoon cover on all incubators, mainly to reduce light and external stimuli. Newborns living in incubators make nests out of cartoon thin bags, which makes every child feel comfortable, shows their love and care everywhere, and treats every child as his own.
Fourth, learn standardized training and services.
Hardware is the foundation and software is the soul. "There are many brands of instruments in neonatology department, including various neonatal ventilators, high-frequency oscillation ventilators, imported and domestic infant incubator, radiation tables, CPAP, central monitors, infusion pumps, infusion peristaltic pumps, etc. Every medical staff is familiar with the operation of each instrument, which benefits from their standardized training. Make rounds and give lectures every day, and learn and improve together in the form of slides and bedside. For new doctors, we also arrange special people to teach, and the weekly training starts from the department system and all the most basic operating procedures, so that everyone can get familiar with and master it faster.
Learning is very tiring, because there are many things to learn, you need to do it yourself to master it skillfully, you need to think hard to adapt to the role change, and you need to cultivate your own cautious and independent spirit. This is a process, stick to it and you will have a perfect jump. This precious learning opportunity has given me a lot of experience and feelings. Perhaps such a learning opportunity will not bring earth-shaking changes to my work, but I have more thoughts on my life and work, hoping to bring new ideas to my future life. Generally speaking, in this rare study, I have benefited a lot from my own efforts and the selfless help of my teachers. In the future work, I will use my proficient business level and people-oriented communication skills to serve more newborns and truly apply what I have learned!
Pediatric learning experience 5 It has been half a year since I studied in a blink of an eye. Looking back on the past six months, there are gains and losses. When I first came, everything was so strange, I was not familiar with the hospital environment, the surrounding routes, the food here and the weather here. Every day of further study feels long. The two people we came to study together this time are both mothers. When we first came here, we all missed home and children. But the people's director, the head nurse and everyone in the department here made us feel the same enthusiasm as here in summer, which made us familiar with the ward environment and workflow, adapted to the life and work here, and established a harmonious relationship with them. Therefore, the later days passed quickly, with different gains and enrichment every day. Because this learning opportunity is very rare, I learned advanced nursing concepts and techniques here, and felt their team spirit, rigorous work style and enthusiasm. glad you came.
First, scientific management.
First of all, there is good teamwork and a diversified echelon of nursing staff. The department implements hierarchical management. There are three head nurses and more than 80 nurses. Wards are divided into 1 1 nursing group. Each group consists of five to six nurses with different qualifications, and is headed by a team leader. If it is not scientific management at different levels,
Richard, what should I do? Although the division of labor is clear, everyone cooperates with each other while completing their respective work. The team leader is responsible for knowing the situation of the children in the ward like the back of his hand, and assigning nurses with different seniority and working ability to take charge of different wards according to the different situations of the patients in the ward, so that the children can get comprehensive and meticulous care. If there is a problem, the team leader can handle it in time. Secondly, hospital infection control is also very good. The ward is divided into different areas. Infected people and non-infected people are strictly treated in separate wards, and those who are identified as infectious diseases are isolated. What impressed me most was that the medical staff washed their hands. The doctor makes rounds every day. The first thing to do is to wash hands. When a patient touches another patient, he will definitely wash his hands first. Every ward has a sink and hand sanitizer, and every child has hand sanitizer beside his bed. Needless to say, nurses wash their hands before each operation. Although they are busy with their work, they will not omit this link. Hand washing has become everyone's habit, because everyone has deeply realized that hand washing is the cheapest measure to prevent hospital feeling, but it is the most effective.
Second, warm and meticulous nursing service
What impressed me the most was that they didn't pay lip service to the humanized service, but actually did it, and truly cared for the children. If CPAP is used, a protective film pad will be used.
Prevent squeezing between nose and stuffy nose. When tearing the tape on your face, lubricate it with paraffin oil before tearing it. The action is very gentle. The children here will insert the stomach tube from the mouth, not the nasal cavity, because they don't want to lose a nasal cavity because of the stomach tube, which will aggravate the difficulty in breathing. For premature babies, they have a special area for premature babies. The environment is quiet, and the sound and operation are as light as possible to reduce the stimulation of sound. Moreover, there is a cover on the incubator, and all treatments are focused on reducing light and external stimuli. Every premature baby has made a bird's nest out of clothes, which makes every child feel very comfortable. If there are special circumstances, they will hang labels and write warm words. If there are some children who need to be isolated, such as children with syphilis, draw some plum blossoms beside their beds and call them "Mei Mei". There is no discrimination at all, but more care. Children with hepatitis B will draw three suns and call them "beautiful sheep" or "pleasant sheep". There are also examples such as "My skin is tender, please take good care of it" and "My hairstyle is handsome, please take good care of my hairstyle", which reflect their love and care everywhere and treat every Mao Mao as his own child.
Pediatric advanced experience 6 Hello everyone. I am a professional newborn nurse. It is a great honor to be sent by the hospital leaders to study neonatal pediatrics in Shengjing Hospital of China Medical University for half a year. At that time, I thought, I must study their advanced nursing technology seriously before I can go to glad you came. In the past six months, I deeply felt that besides learning nursing technology, advanced hospital management mode, thoughtful occupational safety protection and nursing concept are all worth learning. Summarized the following aspects.
First, humanized management:
First of all, there is good teamwork, multiple nursing staff echelons, and the nursing team in the ward is equipped with head nurses, specialist nurses and clinical nurses. Everyone cooperates with each other while completing their own work to ensure the quality of nursing. Ward assistant, responsible for the inspection of out-of-town children and the work of out-of-town chores, hygienist, responsible for ward hygiene (requiring spotless effect), through careful staffing, everyone does his job and everything is under the care of medical staff. Medical safety pictures are posted in the corridor of the ward, instructing employees how to protect themselves at work to prevent accidents, wear gloves when touching blood and body fluids, handle needles and sharp tools carefully, wash hands frequently and avoid cross infection. During my study, I learned that Shengjing Hospital also trained the quality of its staff. Head nurses provide corresponding learning opportunities according to the situation of nurses, organize various types of learning classes, and improve the quality of staff. For the clinical audit of nursing staff, the head nurse said the most classic sentence: "The nurse made a mistake, not a big problem, but a system problem. They will make changes and improve the system according to this matter, instead of punishing nurses. Only in this way can problems be reported in time, rather than concealed, which is conducive to improving the quality of nursing and reducing accidents. " The work in the hospital is tense and heavy. In order to reduce the work pressure of employees, hospitals will install music players in wards, psychological counseling and so on.
Second, the advanced equipment and technical level
Scientific and effective instruments and equipment, centered on the sickbed, are equipped with devices such as negative pressure suction, oxygen, compressed air, power supply, etc. on the wall 1m high beside the bed. All kinds of monitoring equipment, central suction and central oxygen supply equipment can be moved at will, which is convenient to use. In addition, the neonatal ward has an experienced nursing team with advanced nursing level, such as PICC, arterial indwelling and duodenal intubation feeding. They have successfully treated many ultra-low birth weight infants, and intensive care patients account for 20%-30%. The survival rate and critical care success rate of ultra-low birth weight infants below 65,438+500 g have been known by pediatric nurses in hospitals.
Third, quality service.
In the neonatal ward, when nurses treat children, they should first do a good job of communication and appeasement. For example: Baby, aunt will draw your blood, don't move, don't cry, just gently, and it will be fine soon, which not only reduces the psychological damage to the newborn, but also deepens our feelings.
Before the premature infants are discharged from the hospital, parents will be given a lesson on premature infants' nursing, and special nurses will explain and guide them, so that parents can master the daily care of premature infants, such as bathing and feeding, and will not safely hand them over to their parents until they know it. Through standardized and appropriate service methods, care and sympathy are expressed to the families of children, reflecting the service level of hospitals, and service norms are naturally integrated into service awareness and applied to medical practice. The ward has set up a hotline for discharged patients and is responsible for providing all kinds of consulting services 24 hours a day, so that patients can keep in touch with the hospital through the hotline and get professional guidance of medical services at any time.
In a word, through studying in Shengjing Hospital, my skills and thoughts have been obviously improved, and I have full confidence in PICC, duodenal intubation feeding and arterial indwelling needle puncture. In addition, in order to reduce the pain of children, through one arterial puncture, various blood tests were improved, the puncture times were reduced, the success rate was improved, and the blank in our hospital was made up. As a newborn nurse, we should not only have superb skills, but also have advanced nursing concepts to improve the quality of personnel. We should have a sense of responsibility, love and conscience, do every job well, strictly abide by the aseptic operation and disinfection and isolation system, avoid cross-infection and improve the quality of nursing. Be the protective angel of every child.
I think if everyone can enjoy work as life, then the motivation, enthusiasm, sincerity and so on are all available, the efficiency of work will be obvious, their own value will be fully reflected, and at the same time, they will create greater wealth for shareholders. Finally, I would like to thank the leaders of our college for giving me this precious learning opportunity and giving me a lot of experiences and insights. It will seep into my mind and make me more confident to lead the neonatology department of our hospital to a new level.
In July and March, I came to the first integrated management development model of cardiac surgery in China. In 20xx, I was identified as a key clinical specialty by the Ministry of Health-Cardiovascular Center of Pediatric Hospital affiliated to Fudan University for further study. Cardiovascular center consists of cardiothoracic surgery, cardiology, cardiac intensive care unit, echocardiography room, electrocardiogram room and cardiac catheterization room. There are more than 80 medical staff, and nearly 1000 cases of heart surgery are completed every year. Among them, infants under 1 year account for nearly 50% of the total number of open heart operations. I feel honored to study in China Advanced Cardiovascular Center, and I also feel how heavy the burden is as a nurse in cardiothoracic surgery.
First, feelings
At first glance, pediatric hospitals are far from the big hospitals that appear repeatedly in my mind. There is no crowd outside the hospital. There is no noisy sound in the clinic, cartoon toys can be seen everywhere, and the lounge is decorated like home when you see very warm tips. But when I arrived at the cardiac intensive care unit, I was shocked. Advanced equipment, orderly work, operating on such a small heart child.
Second, moved
A child with TAPVD (anomalous pulmonary venous drainage) weighing less than 3 kg and less than 30 days old needs urgent heart surgery. After the operation, I didn't close my sternum. I can see her little heart beating before my eyes. I have never felt my heart beating so close. Seeing my excitement and curiosity, the professor who operated on the children took the initiative to explain to me the pathophysiology, clinical manifestations, operation methods, postoperative nursing points and long-term effects of such children. Every teacher here * * * * * will repeatedly explain the knowledge points I don't know or don't understand, and give me information and documents. I sincerely admire them and thank them.
Third, feelings
Before I came to Shanghai to study, the only congenital heart diseases I came into contact with were ventricular septal defect, atrial septal defect, patent ductus arteriosus and tetralogy of Fallot. I didn't know until I came to study that congenital heart disease is a very simple disease. There are children with complicated congenital heart disease from all over the country. Pathophysiological changes of children with congenital heart disease, postoperative monitoring, family health education after discharge and long-term follow-up after operation are the focus of my study in the past six months. It is hoped that through this study, the perioperative nursing care of children with congenital heart disease will be more comprehensively understood and mastered. It can also be used in future clinical nursing work, and can also drive nursing colleagues in departments to master more knowledge of congenital heart disease.
At the same time, I thank the nursing sisters in cardiothoracic surgery. With your support, I can study outside with peace of mind.
