How to write a sick note for children?

I am the parent of XXX, a student in your class. I am writing this letter to ask you for leave, because my child XXX needs to rest at home for a while because of illness.

First of all, I want to explain the child's illness to you. Last night, the child began to have symptoms of fever, cough and sore throat. I took him to the hospital immediately, and the doctor confirmed the child's upper respiratory tract infection after examination. The doctor advised the child to rest at home and prescribed some medicine for the child. In order to ensure the child's health and safety, I decided to let him rest at home until his illness fully recovered.

Secondly, I want to assure you that the child will recover as soon as possible and go back to school. I have given my child medicine on time according to the doctor's advice, and paid attention to keeping indoor air circulation so that the child can have enough rest and food.
