1. Application Form for Changing/Cancelling Corporate Short Message Service Products of Bank of China Limited, which is signed by its legal representative/person in charge and stamped with the official seal of the company, and check the corresponding options.

2. Power of Attorney (if necessary), with the official seal of the unit and the signature and seal of the legal representative/person in charge.

3. The original and photocopy of the identity documents of the legal representative or the person in charge of the unit and the authorized person.

4. If the SMS receiver is added or changed, the original and photocopy of the ID card of the added/changed SMS receiver shall be provided.
