PICC health insurance claim application

The materials needed for PICC's compulsory insurance claim mainly include power of attorney (in duplicate), PICC claim application form (in duplicate), accident certificate (traffic police accident handling book, accident safety committee, police station, property certificate and unit certificate), loss list, invoice for repairing accident vehicles, repair list corresponding to the lost position of accident vehicles (i.e. material list), driver's license and driving license.
